How to Order
Due to the numerous products we offer, we have a variety of order forms available to provide you with a convenient method to purchase via email or snail mail. All of our order forms are listed below.
If your product requires an ear impression for a custom fit, we suggest the following steps:
Visit with Active Ears at Bluegrass Indoor Range or at a local gun show. You can also call us at 502-876-6640 to arrange a fiting on your schedule.
Visit your local audiologist, ENT clinic or hearing aid dispenser. Many of these businesses offer impression services for a small fee. Refer to the Yellow Pages under "Hearing" and make an appointment with a local audiologist, ENT clinic or hearing aid dispenser. Once you have your impressions, mail them along with a completed order form stating your physical address and method of payment to Active Ears. If mailing a check, please make payable to Active Ears.
For non-custom fitted products, or products you don't see listed on a form, just send us an email telling us what product you are interested in and include your name, address and phone number. We'll get back to you ASAP to gather your payment information.

Music Monitor and Earplugs Order Form
Includes Custom Musician's Earplugs, Custom Music Monitors and other related products.
Custom Electronic Order Form
Includes E.A.R. Digital Series and Private Ears™ Wireless Communications Systems.
Standard Electronic Order Form
Includes SHS, E.A.R. Mini-Canal™ and Open-Ear Series Electronic Earplugs.
Non-Electronic Order Form
Chameleon Ears and all of our Specialty Ear Molds.
Cellular Systems Order Form
Includes Ear Bud Systems, Plantronics and Starkey EarSets.
Accessory Order Form
Includes Foam Ear Plugs, Ear Muffs, Insta-Putty, etc.
ESS Goggles and Glasses Order Form
Eye Safety Systems is a leader in vision protection. We are an authorized dealer of ESS goggles, glasses and sunglasses.
Tactical SHS Order Form
Includes Tactical SHS SWAT Earpieces, Custom Earmolds, and Specialty Earmolds.
Since many of our clients are active in numerous backgrounds and hobbies, we are often asked to make additional products from their original ear impressions. If you have additional product requests like a Cell phone Ear-Bud or Chameleon Ears, please make a special note on the order form and we will be sure to get them done along with your original order.
We do our best to expedite all orders within 7-10 business days. If you require rush delivery, please specify. (Note: All rush orders require an additional $50)
